Application form

The enclosed application form serves as a standard template that all clients are required to complete and sign. This can be done either through traditional mail or electronically via E-signature. Once filled out, the completed form should be returned to the administrative email address provided for processing.

Dear Candidate

Our recruitment process is now being done virtually, once an application form has been completed and all relevant documents listed below received, we will arrange a video interview.

On the day of your interview, we will need you to bring the following documentation:

·       Application form

·       Signed declaration (last page of this application)

·       Up to date CV

·       Educational certificates

Identification and Right to Work documents needed are:

·       In date Passport

·       Visa (if applicable)

·       NI proof (NI Card, HMRC Letter or DWP Letter. We cannot accept pay slips or P45 as proof of NI).

·       Driving license

·       2 X proofs of address (dated within the last 3 months – Electricity/water/gas/landline telephone bill, bank/credit card statement or a council tax bill). Please note – we cannot accept mobile phone bills or TV licenses as proof of address.

·       Occupational Health form completed and signed. (Proof of all immunizations from a doctor or from a hospital) NOT ESSENTIAL.

·         DBS certificate. Must be in date or on the update service.

·         COVID-19 vaccination certificate 

·       2 X passport sized photos

·       Marriage/Birth certificate or change of name document

If you can bring all of this documentation with you on the day, it will help with the speed and accuracy of your registration.
